Once upon a time, long long ago, there were five young ladies who were called the Spice Girls. One of them, born Victoria, was given the kind nick of Posh (a word meaning classy and elegant which she was until she opened her mouth and spoke). Now, her claim to fame is that she’s married to a footballer who can’t kick the ball in a straight line. Anyway, Posh is prone to get acne - not quite what you would expect of someone so, well, Posh. But she’s finally found the cure. While the rest of the world fights acne with Accutane, the footballer’s wife is mainlining natto. This is a favorite of the Japanese who make the stuff by fermenting soy beans. Guess what? It’s full of essential vitamins that fix scarred skin. Natto has a taste that’s best not described and it’s best approached from up wind because it’s not overflowing with natural perfume. Back in the good old days when people really were posh, they used to go to spa towns to take the waters. That also tasted vile and smelled worse, but the classiest could make it look like they were sipping champagne. We’re all waiting for pictures of Posh downing her beans. For those of you not into has-beens, there’s still Accutane for acne. No smell and swallowed whole so no taste problem but, hey, you can’t have everything unless you’re either rich or Japanese.

In some entertainment shows on TV and in the cinema, vacuum tubes are sometimes introduced as a joke at the expense of one of the men. In one sense, this use of embarrassment is one of the usual examples of humour of the unkind, but we can never deny the recognition factor. Almost every adult in the audience seeing the image would understand the function. These devices have entered the public consciousness.

Their success in part is due to the simple fact that they work without having to rely on penile injections which some men find intimidating, and surgery which can be the most expensive option.

So how and why do they work? Well, we need to go back to the mechanics of an erection. When blood is pumped into the penis and the veins to allow it to flow out are constricted, the heart pumps the penis up in much the same way as air or fluids are used to inflate any closed system. The more blood that enters the system, the harder the erection becomes. The vacuum device works on exactly the same principle except that the erection is created by drawing the blood into the penis rather than by a pumping action.

So now we enter what may at first sight seem a slightly surreal world of technology. You take the hollow tube, usually made out of clear plastic and insert the latex constriction band into the base. Now, place the device over your penis and press it against your body to create a seal. Next, we have to create the vacuum. The actual method depends on the sophistication of the device you have bought. The simple mechanical models have a hand pump or squeeze bulb which slowly extracts the air. Others have a battery-driven pump. As the vacuum develops, the penis physically expands which draws blood into it. It takes about three minutes of exposure to this vacuum to draw enough blood into the penis to create a hard erection. The latex ring is then detached from the base and this constricts your penis to prevent the blood from simply flowing back out into your body. Depending on the model, this can require some degree of physical dexterity. The better devices have a “hands-free” system for releasing the ring.

Once the ring is in place, the air can be allowed back into the device, and the device can be removed. The ring maintains the erection for about twenty minutes. In reality, the erection can be maintained for longer but as time extends, so the risk of damage to the penis increases. Twenty minutes is a safe limit.

So is this a good device for you? Well, the scientists who did all the research (a fascinating area of medical research to pioneer and place on your resume) found that this works best in men who retain some ability to get an erection naturally. In fact, if you get a full erection but it deflates soon afterwards, you can use the latex constriction bands to maintain the hardness. Alternatively, you and your partner should engage in whatever foreplay you find works best to get your maximum natural response. Then use the device. Unlike the use of oral medications like Viagra, Cialis and Levitra, this piece of equipment cannot be concealed. It can only be used when you and your partner are completely comfortable with it. You may need joint counselling so that it can be integrated into a sexual routine for you both to enjoy. As it is, some couples do find it interferes with foreplay as it builds to what should be a natural conclusion.

But, you are shaking your head, dismissing this as a clumsy thing to have in your bed. But consider. This is cheaper than surgery or the long-term use of injections. It is completely non-invasive. You are not injecting yourself. You are not spending time on an operating theatre table for a prosthesis to be installed. This is a simple-to-use device that creates an erection in almost every situation in which erectile dysfunction occurs (it is only inadvisable to use this device if you have problems with blood clotting or you already take blood thinners). You can use it whenever you want, as many times as you want without any side effects. The only slight disadvantage is that the erection is only created above the constrction band. Unless you get the ring sited close to the body, there can be a certain lack of overall rigidity. You also have to remove the band before you can ejaculate and allow the semen to pass after orgasm.

Now here is a question for you. When you reach for a tablet of Ambien, do you have performance anxiety - in the nicest possible sense of the words, of course? It seems that a number of people are finding it difficult to swallow their Ambiens (and others medications). In some, this is a psychological problem. Because the patient resents the idea of being medicated, the resistance is increasingly demonstrated through an inability to swallow the “offending” pills. In others, it is a taste issue. Even though the tablet may only be on the tongue for a few seconds, this is enough to trigger a gagging response. Unfortunately, manufacturers make an effort to flavour medications which are in a liquid form, but only rarely do so when they expect the pill or capsule to be swallowed.

There are also people whose underlying conditions may make swallowing more difficult or whose digestive systems may react adversely to the medications. For example, if you need to give a medication to either an autistic child or one who is hyperactive, actually getting a swallowing response may be challenging. For different reasons, if you already have an anxiety or panic condition, you may be predisposed to find some physical activities more alarming or be unable to swallow during an attack. If you have reduced saliva, or problems with your throat, the physical action of swallowing anything may be difficult even if you attempt to wash down solids.

In some cases, it may be safe to crush the tablet and to mix the resulting powder with a strongly flavoured drink or puree (this is not recommended with Ambien, particularly in Ambien CR extended-release form, because it can release too much of the active ingredients into your blood stream too quickly). Obviously crushing overcomes the problem of swallowing a physical object, but it may not solve the problems of taste. Some people can still taste an unusual flavour in any masking medium.

So here comes US-based MonoSol which acquired the technology from Kosmos Pharma in January 2004, and now specialises in the development of edible film strips which are designed to dissolve quickly on the tongue to deliver the medication. The film strips look like postage stamps and they are already used both in cough and cold remedies, and in breath-freshening strips. The delivery system is more convenient than using liquids which can be spilled, and more easily administered as a pleasant-tasting lozenge to improve patient compliance. Unlike some other dissolving systems, the MonoSol film is not fragile.

MonoSol is currently working to complete development of a film delivery system for Ambien. The physical development work is now complete, and the company is currently conducting a pilot trial of a thin-film version of Ambien to establish that this system delivers a dose that is the bioequivalent to the tablet form. The company expects a successful outcome - it has already successfully tested a version of Zofran, an antiemetic used post-surgery and in cancer therapy. The only difficulty arises from the financial climate. MonoSol has just cancelled an IPO intended to raise around $65m to fund research and development. Nevertheless, it is expected to continue development of the current projects including Ambien.

So if you are finding it difficult to swallow your Ambien, relief may be on the way. There is only one final thought. Ambien currently works to induce sleep in around fifteen minutes. Dissolving systems work rather more quickly. If you do decide to use the film system when it arrives on the market, make sure you are lying down when you take your first Ambien film.

Every year, the pharmaceutical industry in the USA spends billions of dollars on direct-to-consumer advertisements for medications that are only available on prescription. In other countries, such advertising is often banned. Advertising to the medical profession is, however, universal - companies need to inform doctors of the products and their use.

It is easy to dismiss advertising as having little effect on consumer behaviour which is, of course, why companies spend so much on advertising campaigns. In the medical field, the adverts are intended to achieve specific goals:

to increase brand name awareness;

to accept links between everyday experiences and medical causes; and

to believe that the branded medications will remove the causes and return you to normal (whatever that is).

The first adverts appeared in 1981 but, in 1983, the FDA imposed a moratorium to research the effect of the adverts on the public. In 1985, the moratorium was lifted because of fears that it was interfering with freedom of speech. Now adverts are permitted so long as they provide a “true” and “balanced” view of the product and its effectiveness. The FDA monitors compliance and can instruct a company to withdraw any advertisement it feels does not comply. Interestingly, some pharmaceutical companies now spend more on direct-to-consumer advertising than doctor advertising. Their view is that consumers can affect doctor behaviour.

In the area of erectile dysfunction, the advertising for cialis runs through a number of different forms. Like any prescription medication, it is aimed at relieving the symptoms of a medical condition. The wording proclaims, “It works!” as if that explains all the benefits of the medication.

These simple statements are often combined with personal testimonials and confirmation that use of the medication avoids the feared outcome, in this case, a failure to penetrate or maintain the erection after penetration. This tends to be the core appeal. If erectile dysfunction is the problem, cialis helps you to function normally again. The medication is packaged as a means of emotional security. Trust the pills to get you back to normal. People are praised because they cared about themselves and the effect their condition was having on their partners. By introducing others as victims, guilt at not getting the “cure” is increased - cialis is life-enhancement for both partners. To support this, people are encouraged to begin the art of self-diagnosis and health management. The adverts give a few symptoms and suggest that those symptoms represent a disorder that can then be “cured”. It is uncommon to find the price of the medication mentioned, or any information about whether it is available on health insurance plans.

There is no doubt that this advertising educates the public about their options when faced by health problems and teaches them to believe that taking a pill is the best solution. When that medication is only available on prescription, it promotes business to the doctors. In the case of cialis, it is interesting to note that the FDA has never queried any of the adverts whereas there has been litigation alleging misleading advertising for competing medications. Add to this the fact that the word-of-mouth advertising for cialis has been and remains very strong, and it is easy to understand why this medication has taken so big a slice of the market so quickly.

Note the level of tension you’re feeling. Then place one hand on your abdomen right beneath your rib cage.
Inhale slowly and deeply through your nose into the bottom of your lungs; in other words, send the air as low down as you can. If you’re breathing from your abdomen, your hand should actually rise. Your chest should move only slightly while your abdomen expands.

1.	When you’ve taken in a full breath, pause for a moment and then exhale slowly through your nose or mouth, depending on your preference. Be sure to exhale fully. As you exhale, allow your whole body to just let go (you might visualize your arms and legs going loose and limp like a rag doll).
2.	Do ten slow, full abdominal breaths. Try to keep your breathing smooth and regular, without gulping in a big breath or letting your breath out all at once. It will help to slow down your breathing if you slowly count to four on the inhale and then slowly count to four on the exhale. Use this count to slow down your breathing for a few breaths, then let it go. Remember to pause briefly at the end of each inhalation.
3.	After you’ve slowed down your breathing, count from twenty down to one, counting backwards one number with each exhalation. The process should go like this: Slow inhale … Pause … Slow exhale (count twenty) Slow inhale … Pause … Slow exhale (count nineteen) Slow inhale … Pause … Slow exhale (count eighteen) and so on down to one. If you start to feel light-headed while practicing abdominal breathing, stop for fifteen to twenty seconds and breathe in your normal way, then start again.
4.	Extend the exercise if you wish by doing two or three sets of abdominal breaths, remembering to count backwards from twenty to one for each set. Five full minutes of abdominal breathing will have a pronounced effect in reducing anxiety or early symptoms of panic. Some people prefer to count from one to twenty instead. Feel free to do this if you wish.

If you cannot get to sleep and it happens everyday for weeks, ambien is the answer!

Under tension, your breathing usually becomes shallow and rapid, and occurs high in the chest. Shallow, chest-level breathing, when rapid, can lead to hyperventilation. Hyperventilation, in turn, can cause physical symptoms associated with anxiety, such as light-headedness, dizziness, heart palpitations, or tingling sensations. When relaxed, you breathe more fully, more deeply, and from your abdomen. It’s difficult to be tense and to breathe from your abdomen at the same time. By changing your breathing pattern from up in your chest to down in your abdomen (stomach area), you can reverse the cycle and transform your breathing into a built-in tool for anxiety control.

Abdominal breathing triggers a host of physiological transactions that promote relaxation and diminish anxiety. Listed below are some of the benefits of abdominal breathing that translate into greater relaxation and lowered anxiety.

Increased oxygen supply to the brain and musculature.

Stimulation of the parasympathetic nervous system. This branch of your autonomic nervous system promotes a state of calmness and quiescence. It works in a fashion exactly opposite to the sympathetic branch of your nervous system, which stimulates a state of motional arousal and the very physiological reactions underlying panic or anxiety.

Greater feelings of connectedness between your mind and body. Anxiety and worry tend to keep you up in your head. A few minutes of deep abdominal breathing will help bring you down into your whole body.

More efficient excretion of bodily toxins. Many toxic substances in the body are excreted through the lungs.

Improved concentration. If your mind is racing, it’s difficult to focus your attention. Abdominal breathing helps to quiet your mind.

Abdominal breathing by itself can trigger a relaxation response.

The exercises below will help you change your breathing pattern. By practicing them, you can achieve a state of deep relaxation in a short period of time. Just three minutes of practicing abdominal breathing or the calming breath exercise will usually induce a deep state of relaxation. Many people successfully use one or the other technique to abort a panic attack at its first stirrings. The techniques are also very helpful in diminishing anticipatory anxiety you may experience in advance of facing a fearful situation or in easing everyday worry.
